We’re excited to introduce a new built-in report template that aligns with the official NPMA-33 format, the standardized form approved by HUD for federally backed mortgages when a wood-destroying insect (WDI) inspection is required. This form is essential for real estate transactions in many states and must be completed with precision, without altering its structure or language.

What Is the NPMA-33 Form?

The NPMA-33 is issued by the National Pest Management Association and is the only accepted document for reporting WDI inspection findings under federal loan programs. It covers the identification of termites, carpenter ants, powder post beetles, and other WDIs, making it a critical part of the home-buying process.

Key Sections of the Form

Our digital NPMA-33 template mirrors the official format and includes the following sections:

  • Inspection Findings – Records visible signs of infestation or damage by WDIs.

  • Obstructions or Inaccessible Areas – List areas that could not be inspected and explain why.

  • Treatment Recommendations – Indicates whether treatment is required, recommended, or not necessary.

  • Conditions Conducive to Infestation – Identifies environmental or structural issues that may promote infestations.

  • Inspector and Company Certification – Automatically includes licensing details and digital signatures.

  • Buyer Acknowledgment – Provides a space for the buyer to sign and confirm receipt of the report.

Template Objectives

Our goal is to eliminate the need for paper forms or scanned documents by offering a streamlined, digital version of the NPMA-33 that is:

  • Accurate

  • Compliant

  • Professional

This enhances the efficiency of pest inspection workflows and ensures your reports meet industry and regulatory standards.

Key Features

Every section is clearly labeled and matches the official form.

  • Non-Alterable Language and Formatting

Complete and finalize reports from your desktop or on the go.

Ready to use the new NPMA-33 template?

Check your report template library available in ISN. Navigate the main menu and select the option 'ISN Report Writer'

Then, select 'Add from library'

Then, click the 'download' button , to add the NPMA-33 template to your library

From that page, you’ll be able to add the template to your library. Please note that this is a specially coded template with a unique layout and format, so editing options will be limited.

Once added, make sure to enable the template so it appears as an option when you initiate your next inspection order in ISN.

To use the template, open your report writer from any order by selecting Actions > Write Report > Add New Template

Then choose the NPMA-33 template.

The process for filling it out is similar to other specialized templates like the 4-Point or Wind Mitigation reports. Just enter the required information in each field, and the system will generate a compliant PDF.
